On the joyful occasion of the baptism of your child, Mt. Barker/Strathalbyn Catholic parish seeks the following information about your child and family as part of updating its usual census records. This privacy policy notice is required these days by amendments to the Privacy Act in 2001.

The census information will only be used to distribute information about parish activities and to visit you from time to time. It will also be the basis for other financial and pastoral activities that are part of parish life.
We keep these details on our church database which is stored in our parish computer and protected by passwords. The database is updated by the Parish Secretary.
Our policy is to destroy the information when it is no longer required, such as when you may leave the parish.

You do not have to complete this form, however, if you choose not to, you may limit the parish’s ability to pastorally care for you. We do need the information on the form to record your child’s Baptism in the Register and to prepare a Certificate of Baptism for you.
